US CareNet launches care management software platform

US CareNet announced the formation of a software platform to address the needs of chronically ill patients.

The national senior care services provider purchased controlling interest in the technology used for its care management subsidiary NavCare, to form a new company HC360 Technologies. The newly-enhanced HC360 Technologies platform provides chronic care management, transitional care management and population health management services to patients as an extension of their primary care.

The Centers for Medicare & Medicaid Services started reimbursing providers who actively manage care delivery for Medicare patients suffering from two or more chronic conditions in 2015.

“NavCare needed a platform that would address the challenges and continuous changes of healthcare requirements,” said Gary Liuzzo, vice president of information technology for US CareNet in a press release. “HC360 Technologies is positioned to deliver an advanced, cloud-based solution with integrated Voice over Internet Protocol (VoIP).”

HC360 Technologies is owned by US CareNet as majority owner and Care 24/7. Augusta, Georgia-based US CareNet offers a group of post-acute health care and senior living services, including home health and private duty agencies, to meet the healthcare needs of aging Americans
