SNF awarded for reducing falls 53% in 2011

A falls management program at The Center for Nursing and Rehabilitation (CNR), Brooklyn, N.Y., resulted in a 53 percent reduction in resident falls last year and recognition by the American Medical Directors Association  (AMDA).

Using a person-centered, interdisciplinary approach, replicable changes were implemented such as installing black toilet seats to improve visual contrast in bathrooms and installing mattress overlays with raised ridges help to prevent users from rolling off the bed.

Staff noticed that falls were more prevalent in the early evening (7-9 p.m.) when residents had more free time before retiring. To fill those evening hours, therapeutic recreation programming was instituted to provide more structure and options for resident engagement.

CNR, a 320-bed not-for-profit facility, is part of the CenterLight Health System.
