Single-platform mobile personal health record unveiled

An all-in-one solution that allows healthcare providers to integrate the latest mobile technologies with an electronic health record (EHR) may not be such a far-fetched idea after all.

Collain Healthcare, an LG CNS company, has launched its Interactive Virtual Care Team, a scalable suite of applications on a single platform that enables real-time coordinated care across the entire care continuum.

The development of the IVC Team platform marks the first time that a mobile personal health record (mPHR) has combined telehealth, remote patient monitoring, medication management and clinical decision support onto a single, integrated platform that can connect to EHRs and health information exchanges (HIEs), according to a company press release.

Vital signs and other patient data are collected with Bluetooth-enabled devices and apps, using either an Apple or Android device. The data elements are automatically uploaded to the cloud where they can be accessed in real-time through a connected EHR or HIE. Since the app can follow the person anywhere, it extends the data-gathering reach through transitions of care and home-based or community-based settings.

And because patient information is delivered in real-time, healthcare providers and caregivers can intervene quicker if there is a problem and even communicate directly with the patient via telephone or video conferencing.
