Post-acute care improvement is goal of Golden Living, naviHealth partnership

Golden Living and naviHealth have partnered in an effort to enhance post-acute care service delivery for people admitted to Golden Living’s skilled nursing facilities (SNFs) around the country. The clinical and strategic collaboration will use naviHealth’s decision support technology, post-acute care analytics platform and network of clinicians to try to enhance outcomes and improve the transition of care for patients who wish to return home.

“Our partnership with Golden Living to deliver comprehensive care management services to its eligible SNF providers is, hopefully, the first of many such alliances we can forge to improve the delivery and management of post-acute care across the country,” Clay Richards, naviHealth president and CEO, said in a statement.

Program elements of the partnership include the sharing of data, clinical collaboration on emergency department diversion and readmission programs, and the appropriate inclusion of Golden Living providers in any proprietary provider networks established by naviHealth.

“We believe that our collaboration with naviHealth will bring significant value to the patients within our provider network,” Neil Kurtz, MD, Golden Living president and CEO, said in a statement. “The partnership between naviHealth and Golden Living is one example of how providers and payers are working together in response to our country’s healthcare reform.”

There are 295 Golden LivingCenters in 21 states, and Golden Living also offers assisted living services at 12 of its locations. Golden Living companies also provide services to more than 1,000 nursing homes, hospitals and other healthcare organizations in 40 states and the District of Columbia.
