Intel-GE Care Innovations, Amedisys ink home health deal
Intel-GE Care Innovations, Roseville, Calif., has partnered with Amedisys, a Louisiana-based home health and hospice services provider, to offer Care Innovations’ Health Harmony platform to patients at home, hoping to reduce readmissions and allow providers to work together on care interventions. The population base will include Amedisys’ client range, which spans 34 states.
Remote care management (RCM) programs will focus initially on people who have chronic illnesses known for high risks of complications and readmissions, including congestive heart failure, chronic obstructive pulmonary disease, diabetes and depression. The RCM programs will be tracked for outcomes and the data analytics will be used to develop new best practices for individualized remote care plans and provider collaboration, notes a Care Innovations press release.
News of the deal follows on the heels of last month’s announcement of Care Innovations’ five-year partnership with the University of Mississippi Medical Center to combine state population health management initiatives with remote care management platform to improve the management of expensive and tricky chronic conditions across the southeastern United States.
