HIE tool for LTPAC providers

As the June 15 deadline for grant applications approaches, Salt Lake City-based VorroHealth announced that its KeyHIE Transform solution is included on the short list of tools that long-term and post-acute care (LTPAC) providers can use toward $1 million in grant money offered through the Office of the National Coordinator for Health Information Technology's Community Interoperability and Health Information Exchange (HIE) Program.

ONC’s program will assist up to 10 community organizations, state or local government agencies, or other community groups in the sharing of health information and coordinating care across an HIE.

VorroHealth, formerly BlueStep/BridgeGate, developed its KeyHIE Transform solution so that LTPAC providers can easily transmit resident health information directly to an HIE.

Working with or without an electronic health record (EHR), KeyHIE Transform generates assessment information from a SNF's Minimum Data Set (MDS) system or from an HHA's Outcome and Assessment Information Set (OASIS) system and then converts that data into a Continuity of Care Document (CCD/C-CDA) which contains all the assessment information.
