CareCentrix partners for home care patient portal
CareCentrix, Hartford, Connecticut, an industry leader in making the home the center of patient care, has entered into a strategic alliance with Medullan, a leader in digital health experience design and patient engagement. The relationship will focus on developing virtual care tools that connect providers, patients and payers through flexible, mobile technology while the patient is outside the four walls of the hospital. This is one of several alliances that CareCentrix is unveiling to create the new ecosystem for post-acute care management.

The alliance marries Medullan’s digital expertise and care experience platform, VARA, with CareCentrix’s HomeBridge technology, resulting in a seamless digital experience for patients and post-acute providers, which will be personalized with information to optimize patient care.
