And the 2015 OPTIMA Award winner is…
The competition was fierce among the program submissions for the 2015 Long-Term Living OPTIMA Award, but the external panel of judges ranked one program above the rest. Congratulations to Silvercrest Center for Nursing and Rehabilitation, a 320-bed skilled nursing facility in Briarwood, N.Y., for its cross-department program to reduce hospital readmisisons among chronically ill residents, many of whom are ventilator-dependent or are tracheostomized.
