Collaboration tackles chronic care management
San Diego-based Qualcomm Life Inc. and New York-based AMC Health have announced a collaborative agreement to develop a connectivity platform that will improve care coordination for those living with chronic diseases. The initial focus will be on heart failure, hypertension, diabetes, asthma and COPD.
The collaboration will integrate Qualcomm Life's 2net connectivity platform with AMC Health's CareConsole care coordination platform.
Biometric data captured from medical devices through Qualcomm's 2net platform, hub and mobile solutions will be transmitted securely to AMC Health's CareConsole platform where they will be analyzed and made available to clinicians via a secure web portal.
