COVID-19 Survey for Senior Care/LTC Facilities

You are invited to participate in a short survey on the impact of COVID-19 on Senior Care and Long Term Care Facilities. 

No one understands the challenges you face during this pandemic the way you do. 

Our goal for this survey is to collect and share the concerns and insights of senior care leaders like you from across the country. This survey is a follow-up on the research completed in March and June to capture new information and also compare how concerns have changed over the months.  We will also examine the challenges of reopening and how facilities are balancing the need for residents to socially engage while keeping safe from COVID-19.

At the end of the survey you will be asked for your contact information so that we can provide you with the results of the survey and analyze responses on a regional as well as national level.

Your responses will be strictly anonymous. Responses will be analyzed and shared only in the aggregate.
